The Children's Ark Intake Process

Intake Process for Children

The Intake process will be initiated by a telephone call from the referring agency such as the Juvenile Probation Department or Children and Youth Services; FSP, Clinical, Medical and Educational documents will be obtained during the intake process. All necessary releases of information will be obtained at this time.

The referring agency may request to arrange a two-night or longer pre-placement visit at the residence. The county case worker or probation officer will provide the necessary documentation at the time of pre-placement.

At the closing stage of the visit, a determination will be made as to the suitability of placement. Should a decision be made for placement, an intake packet will be completed. All files are kept in accordance to HIPAA regulations.
